Explain lead outreach state

/v1/outreach-campaigns/:campaign_id/leads/:lead_id/explain

Dry-runs the outreach planner for one lead in a campaign and returns the decision trace: eligibility checks, run and attempt state, held slots, channel capacity, pacing, and the gate currently blocking progress. This endpoint is read-only and does not create attempts, reservations, invitations, or messages.
